Job Description:
HCA Healthcare is seeking a dedicated RN Navigator
Cardiovascular to join our team of compassionate healthcare
professionals. In this pivotal role, you will guide patients
through the complexities of cardiovascular care, ensuring seamless
coordination among healthcare providers and enhancing patient
outcomes. You'll serve as an advocate and educator for patients,
assisting them in making informed decisions about their health
care, monitoring their progress, and supporting them throughout
their treatment journey. Act as a primary point of contact for
cardiovascular patients, guiding them through the healthcare system
Coordinate multidisciplinary care plans with healthcare teams
Educate patients and families on disease management, treatment
options, and healthcare resources Monitor patient progress and
adapt care plans as necessary Facilitate timely scheduling of
tests, appointments, and procedures Advocate for patients' needs
and preferences across healthcare settings Participate in quality
improvement initiatives to enhance patient care Strong
interpersonal and communication skills Ability to manage multiple
tasks efficiently Deep knowledge of cardiovascular diseases and
treatment protocols Proficient in using medical software and health
information systems Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Active Registered Nurse (RN) license
in the state of practice Certification in Basic Life Support (BLS)
Certification in Advanced Cardiovascular Life Support (ACLS)
