Job Description:
Agape Care Group, a leader in hospice and palliative care across
the Southeast, is looking for a compassionate and dedicated
Registered Nurse (RN) to join our team. In this role, you will have
the privilege of providing comprehensive nursing care, focusing on
the comfort and dignity of patients in their final stages of life.
Working closely with patients and their families, you will play a
pivotal role in crafting a supportive environment tailored to
individual needs, guided by our mission to enhance the quality of
life for those at life's most sensitive juncture. Provide
professional nursing care for assigned patients in a hospice
setting. Assess, plan, implement, and evaluate patient care plans
in collaboration with the interdisciplinary team. Administer
medications, maintain patient records, and document care provided.
Educate patients and their families on symptoms management,
end-of-life care, and healthy behaviors. Participate in development
and revision of the patient care plan. Regularly communicate with
the care team members to coordinate and optimize care. Respond to
emergency situations promptly and effectively. Excellent patient
assessment skills Strong communication and interpersonal skills
Proficient in using healthcare software and systems Ability to
provide emotional support to patients and their families Knowledge
of pain and symptom management techniques Capacity to work
independently and in a team environment Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Valid RN license in
the state of employment. CPR certification. Certification in
Hospice and Palliative Nursing (CHPN) preferred but not
required.
